Association of Supervisors, shipyard handstamp postmark.
Lower: Commissioning cachet by Robert Ekas, ship's cachet,
shipyard handstamp postmark, commanding officer's autograph and
Alabama Governor George Wallace's autograph.
Commissioned August 26, 1964.
Operated in Pacific from Apra Harbor, Guam 1965-1970.
Joined Atlantic fleet 1970 after conversion to Poseidon missile system.
From 1972-1978 operated out of Holy Loch, Scotland.
Operated out of King's Bay, Georgia 1980.
Converted to Trident missile system 1987.
Decommissioned February 5, 1995.
Scrapping/recycling completed October 13, 1995.
